RN, BSN, Case Manager

As part of our CCM program patients will meet with our case manager. Deb has worked as an RN for 35 years, 5 years as a case manager and 20 years as a nurse educator. Her role includes assessment of patients' medical and psychosocial needs to develop an individualized care plan with primary care providers and specialists. She can assist patients and families in arranging medical services, treatments, diagnostic tests, therapies, and medication management. A large part of Deb's role is to educate patients and their families about their medical conditions, treatments, and self-management techniques. She will be able to monitor patients' progress with follow-up visits and calls. She will be able to assist patients and families in accessing community resources, services and programs that can support their healthcare needs. Case managers can play a pivotal role in improving patient outcomes, reducing healthcare costs, and enhancing the overall quality of care.
